What is a Health Home?
A health home is a care management model formed by the network of an individual’s service providers and through which they can communicate with one another to provide an individual with more comprehensive care. This is done through a care manager, who will help to oversee your care. The goal is to increase quality of care and decrease Medicaid costs through reducing emergency room visits, hospitalizations and duplicated care.
What is Care Management?
Care management is a FREE service offered by Fountain House that allows you to play the central role in the development of your plan of care, making your wants and needs a priority.
Together with your Care Manager, you will work to create a personalized plan of care that includes your medical, behavioral and social needs. This plan of care will ensure that all of your services are paid for.
Care Management assists with:
- Quality Support Services
- Medical Doctors and Specialists
- Mental Health Providers
- Home and Community Based Services including Psychosocial Rehabilitation, Habilitation, Employment, Education, Peer Supports, Family Supports, Crisis Respite.
Who is eligible?
To be eligible for the Fountain House Care Management, an applicant must:
- Have Medicaid and be Health and Recovery Plans (HARP) eligible
- Have a primary presenting problem associated with severe and persistent mental illness.

How much will this cost?
Care Management is a FREE enhancement to your regular Medicaid services that enables more coverage of behavioral health services.
Will I have to change my doctors?
No. The health home model is person-centered, meaning that the individual makes their own choices in regards to their treatment, such as service providers or goals.
How often do I have to meet with my Care Manager?
You will meet with your Care Manager once a month to make sure your needs are being met and to address any changes you would like to make in your plan of care.
